The importance of compassionate, community-based support has never been clearer. In today’s fast-paced and often isolating world, many people living with mental health challenges find themselves feeling disconnected. That’s where the Mental Health Community Connector Service (MHCC) steps in.
MHCC supports individuals to reconnect with their communities, rediscover their strengths, and take practical steps towards positive wellbeing. By offering personalised, non-judgemental support rooted in real human connection, we aim to empower people to move forward in ways that feel meaningful to them.
Working closely with NHS Primary Care and five local Primary Care Networks (PCNs) – Ely North, Ely South, Cambridge City, Cambridge City 4, and Granta – our service offers a lifeline for individuals who may be struggling with moderate to severe mental health issues. Through tailored, non-clinical, strength-based support, we help individuals build confidence, rediscover their independence, and take meaningful steps toward a healthier future.
What Does MHCC Offer?
The MHCC team is dedicated to holistic wellbeing, supporting individuals in a way that’s personal, flexible, and empowering. Our approach focuses not only on mental health but on overall wellbeing – social connection, confidence, and independence.
The MHCC service is designed to be highly personalised, recognising that everyone’s journey with mental health is different. We take the time to get to know each individual – what matters to them, what challenges they’re facing, and what kind of support would genuinely make a difference in their life.
For some, that might mean working together to set and achieve meaningful personal wellbeing goals – goals that reflect their values, interests, and hopes for the future. Others may benefit from strength-based conversations that help them focus on their existing skills and abilities, rather than their limitations. We encourage people to recognise what they’re already capable of, and build on those strengths to create positive change.
In many cases, support involves helping individuals reconnect with their local community. Whether it’s finding a social group, a creative class, a walking club, or a local support service, we work alongside individuals to explore what’s available and what might spark interest or bring a sense of connection. These small steps can be key to rebuilding confidence, reducing isolation, and supporting long-term wellbeing.
This service is particularly valuable for individuals whose recovery is supported by increased social interaction, which we know plays a vital role in mental wellbeing.
Every person we support receives a service that’s tailored to them – because mental health support should never be one-size-fits-all.
How the Support Works
We understand that everyone’s situation is unique. That’s why the way we deliver this support is also tailored to individual needs, and can include:
- Face-to-face, telephone, or online sessions – whichever works best for the person
- Up to six weeks of support, with a review at the end of that period
- One-hour sessions, offered weekly or fortnightly
- A clear focus on goal setting, ensuring the individual feels empowered and supported in their journey
